CARE (Continuous Assistance, Rehabilitation, and Education) is a unique method of providing services to adults recovering from a severe and persistent mental illness. All CARE services are provided on an outreach basis, usually in the client's home or other community setting. We provide treatment in the least restrictive environment possible, without sacrificing client care.
The CARE program begins by building a partnership between the individual client, appropriate members of his or her family, and other staff of professionals. Clients work with our staff to develop a set of goals, such as living independently, receiving an education, or maintaining gainful employment. We then write a plan to achieve the established goals and begin working the plan. As the client meets their goals the plan is reevaluated and revised.
The backbone of the program is the CARE team. New clients are assigned to a team of professionals made up of clinical staff from a variety of disciplines including therapists, case managers, psychiatrists and nurses. This approach allows for more complete treatment planning, increased client attention, and involvement from a wider selection of professionals. Each client is assigned a primary case manager who is responsible for maintaining an ongoing professional relationship with the client in a community setting. Case managers, along with input from a clinical supervisor and other CARE team members, oversee a variety of responsibilities on behalf of their clients.
- Maintain up-to-date assessments and evaluations.
- Treatment planning and monitoring the clients progress towards meeting goals and objectives of the treatment plan.
- Locate, coordinate, and monitor all necessary medical, social service agency, and psychiatric services.
- Assist clients in developing and executing a plan to manage their finances, including a payee service.
- Help clients find and keep an apartment or other appropriate living arrangement.
